Outstanding Achievement Awards
This award is about celebrating someone who has pushed through change with tenacity and determination. This person may have had to overcome objections or barriers, finding new ways of working, developing new approaches to deliver the best possible care or other impact for people in the community. The winner of this award is chosen from the other category winners.

The Care Hero Award
This award recognises that all those working in care, who make a difference to people’s lives every day and this award celebrates that. Do you know a someone working in care who has worked hard to enable someone to live an independent, safe and fulfilled life? Have they helped to change someone’s life for the better? It is these everyday stories about how our care workers make a difference (big or small) that the judges want to hear.

Carer in the Home
This award is about celebrating the important work that carers in the home do. Are you, or do you know someone who is demonstrating outstanding levels of commitment to patient care and put the patient at the heart of what they do. Does this person go the extra mile?

The Excellence In Nursing Award
We are looking for a health care professional who demonstrates an innovative and caring approach to their work. This person must demonstrate outstanding levels of commitment to patient care and put the patient at the heart of what they do. Entrants can be from any health care profession, and can work anywhere, from community care to in surgical theatres.

Fundraiser of the Year
This award goes to a person who has raised a substantial amount of money for health and care charities over the last 12 months. They could have taken on a challenge or set up a just giving page having been affected or received support from the charity. We want to know how they have raised the funds and how much they have raised.

Best Place to Work Award
This award will go to the company which believes its environment is the best place to work. The judges will be interested in how the company treats employees with respect and equality; how a company encourages employees to take pride in their work and how the atmosphere created in the workplace is conducive to a successful team environment.

Care Home of the Year
We are looking for a Care Home that stands out for the level of care they provide. Have you or your family member received great care? Has a care home workers gone above or beyond the call of duty, or has the receptionist always greeted you with a smile and a friendly word? This award recognises the contribution of everyone working in a care home providing a good service to patients, where staff have gone the extra mile when treating you or someone you know.

Team of the Year
We want to hear about teams working together to deliver improved care. They can be from any sector but must demonstrate impact, very strong team working, strong communication, and the ability to work across organisational boundaries.

Oral Health Care Professional of the Year
This award is for dentists, orthodontists, hygienists, dental nurses, dental practice manager or receptionist or anyone work in oral health care who demonstrates an innovative and caring approach to their work. This person must demonstrate outstanding levels of commitment to patient care and put the patient at the heart of what they do.

GP of the Year
We are looking for a GP that stands out for the level of care they provide. Have you received great service? Has a doctor gone above or beyond the call of duty? Has this GP helped you when you really needed support? We want to hear about GP’S who are at the heart of their community and working hard to help improve peoples health.

Hospital Worker of the Year
This award is for anyone working in a hospital, this could be a doctor, surgeon, receptionist, porter, radiographer or anyone who demonstrates an innovative and caring approach to their work. This person must demonstrate outstanding levels of commitment to patient care and put the patient at the heart of what they do.

Emergency Services ‘Blue Light’ Hero of the Year
This award goes to a person from the ‘Blue Light’ services sector who have gone above and beyond the call of duty and shown great coverage when saving a life. This could be a paramedic or a member of staff from our other emergency services whose interventions have made a real impact on someone’s life.

Health and Wellbeing Award
This award is open to an organisation in any sector, not just health, making health and wellbeing an organisational priority. The judges want to hear how the senior management employee health and wellbeing a core priority and how the company has a health and wellbeing strategy. How have they integrating a health and wellbeing in relevant organisation’s plans policies and communications and empowering a team of workplace health champions with enough autonomy and support to design and implement health promoting initiatives. How is the organisation promoting healthier lifestyles and safety in the workplace and promoting mental health through supportive policies and activities aimed at increasing employee resilience.

GP Practice of the Year
We are looking for a GP practice that stands out for the level of care they provide. Have you received great service? Has a practice nurse gone above or beyond the call of duty, or has the receptionist always greeted you with a smile and a friendly word? This award recognises the contribution of everyone in the GP practice in providing a good service to patients, where staff have gone the extra mile when treating you or someone you know.

Unpaid Carer Award
The Unpaid Carer Award will be given to an unpaid carer who, in the opinion of the judges, has demonstrate that they have had a significant positive impact on the lives of a vulnerable person. Do you know a someone giving up their time to help someone to live an independent, safe and fulfilled life? Have they helped to change someone’s life for the better? It is these everyday stories about how our unpaid carers make a difference (big or small) that the judges want to hear.
